OUTSOURCED CLINICAL & MED MANAGMENT
Our residents’ medication and clinical needs are addressed 24 hours/7 days a week in order to ensure Doctor’s Orders are administered professionally and as prescribed. Our residents have 24 hour access to our Registered Nurse (RN), either by phone of physical visit when needed.

  • Medication and health assessment for each resident is performed every 45 days or as necessary.
  • Open-access for resident’s Physician to visit the resident when medically required to visit, either planned or emergency visits.
  • If a condition warrants a medical specialist, an appointment is made; transportation with a companion is arranged if a family member is not available.
  • Medications are dispensed by certified staff who ensures practice of professional medical ethics compliance.
  • Eye, foot, and dental care are scheduled as required by resident’s physician.

SKILLED NURSING SUPPORT FACILITIES

This level of care support is ONLY facilitated by a licensed medical personnel and provides care to seniors with complexed medical needs that your loved one cannot fullfil on their own. NON-SKILLED FACILITIES are different, these facilities are designed to assist seniors with the basic day-to -day selfcare task such as bathing, eating, and dressing.

ASSISTED LIVING SUPPORT FACILITIES

Is uniquely set up to allow your loved one the opportunity to take advantage of off site outings accompanied with a caregiver while living in a private apartment and still get the help of medication reminders, having laundry taken care of, and receiving meals of their choice MEDICARE COVERAGE - NO LONG TERM INS - YES MEDICAID - SOME COVERAGE

MEMORY CARE SUPPORT FACILITIES

These facilities are our more sort after ones. They take on extra security measures to protect your loved one who is typically suffering from different stages of Dementia, Alzheimer's disease, or any other disease that may target memory loss issues. MEDICARE COVERAGE - NOT USUALLY MAY BE COVERED UNDER CIRCUMSTANCES

RETIREMENT HOUSING

Are houses designed for your loved one if they are relatively healthy ,active, and independent but still may need to be looked upon or seen over by a caregiver while living at a single family residence, apartment, or condominium which may also be age restricted. LONG TERM CARE (HOSPICE)
